Download the attached form, fill it out and print it. Attach it to the products you are returning.

Please return all cuvettes in their original packaging. Cuvettes from different tests should not be packed in the same cuvette box. If cuvettes are returned in packaging other than their original packaging, the sender will be charged for disposal costs, plus a 20% handling fee.

Please note the cuvette groups according to the following table:

Markings and packaging:

Use the original delivery boxes; they already include the necessary markings. Mark the packages with the UN number(s) and warning label(s).

Remove any old labels if necessary. The cardboard box must be UN-approved. Only test packages belonging to the same
group may be packed together. The maximum permissible weight of the package is marked on the cardboard box (e.g., X
18= 18 kg gross). The filling material must be environmentally friendly and absorbent. Pack the cuvette boxes upright in the
cardboard box in the direction of the arrows.

• Used tests must be returned by freight, not by parcel post.
